There is no reason to submit a photo whose longest side is less than 640 pixels. Would've been a 5, but I had to lower your score for posting a small pic, making it harder to see.

"Model" doesn't look very natural and seems too posed and stiff. A tighter crop to eliminate the distracting background and focus more on his emotion would've been more effective IMO.

Looks slightly out of focus and oversharpened to compensate. Good idea, could do with less chain from the top and a slightly tighter crop on the apple and hardware.

Lighting could've been better. A quick autolevels would've improved contrast and eliminated some of the color cast as well.

While this could be painful, I do not get the sense of the hammer being driven into the man's skull. It looks too stationary and setup. Probably photographing the hammer slightly blurred from motion, right before the moment of impact would imply more pain.
