 Focused on mouth / teeth, turned off lights, took photo with bounce flash pointing the other direction so keep mouth dark, move person out the way, remove speedlite flash from camera, crouch model against black background and fire flash to expose the crouching model so as to appear he is in the mouth... end exposure.

It is quite simple really. I had my palms facing straight up (as though I was pushing on the ceiling). It also helped that the flash was fired by hand from a low position (see the knee shadow)so the palms were almost perfectly black. I just happened to get get good positioning after about 40 shots... the other shots overlapped badly or were no where near the teeth. Hope that clears up a few issues.

Honestly - they probably just didn't like the content of the photo. I think it's marvelous and well executed. Grats on your top 10 finish!

Thanks to everyone for the wonderful comments. I am very happy to finish in top 10 seeing it is my highest finish... but I was sectretly hoping for a higher placing :-) Would really love the people that gave me votes of 1-4 to comment so I can see why they gave low scores and try to improve.
