I made the smoke. I never use plug-ins.
You start with a fairly large soft brush on a new layer. Best to make about three of these. You use white to begin to draw the form. You then take each layer and apply a strong gaussean blur. before you combine them you use a soft eraser to help out the formation. You then select the best transparancy and go on to the next layer to make corrections unril you are satisfied.

I like how you figured out your way into manually creating this smoke-like feeling, the blur is a bit too much though in my opinion, and it's also almost impossible (at least to me) to create a 3D effect to it, real life smoke always contains some level of sharp details and it's also distributed in 3d space and not necessarily flat looking.

Also, I second that comment about the lamp.

I think the mistake in this is that the everything is quite nice, but I dislike the lamp. I would re-do this but buy a cheap brass tone genie lamp off of ebay and photograph it and use that instead. It will add a greater semblance of reality.
