 Three light set up. Two soft boxes, one on each side of the face, and one light placed behind the back of the head. I hope there isn't a bunch with this kind of light set up. I know it is not original but I like the mood you can create with it. I will probably get voted down because people won't find it to be a pleasent image. Oh well if it does.

Beautiful symmetry ... probably crop a little closer from the top and bottom for a closer and even more sinister look but I am just being petty. The lighting especially the back lighting of the hair is gorgeous.
