The lighting here was with two 500 Watt Work lights. 1 located to the left and closer to her and one just slightly to the right but at a further distance. There is also a 250 Watt light hanging in front of her from above.
It was shot using my 50mm f/1.8 lens with an HD filter on.
Editing steps are a little fuzzy but here is what I believe I did in photoshop.
-Unsharpened Mask
-Duplicate layer blend mode softlight
-Selected the eyes and used hue/saturation layer with some desat.
-Reselected eyes and used a curves layer to brighten them
-Hue/sat layer and desat
-click on background layer and add another hue/sat layer blend mode color click colorize
-Flatten image
-Channel mix layer click gray scale and adjust red and green
-Flatten image
-Image mode grayscale
-Image mode duotone used black and gold
