Very nice shot! Simple, pleasing, and to the point without needing any words. Perhaps a slightly darker (blue?) background would improve the contrast with the light frosting & figures?

These sexy birthday cards can definitely be fun. However, this one just looks too soft. (The focus, not his abs.) Also, having him hold a birthday cake (or similar) would add to the message.
