Nice shot. The eyes are what make it special. The button on the shirt is a little distracting. Actually it would be nice to have naked shoulders and perhaps similar shaped (to the cotton(?)) dangley earrings.

the smoke being forcefully exhaled would have been cool. The overhead light moved back a bit (if possible) so a little less "washing out" or bleaching of his head. Maybe a foot on a stool. Otherwise cool atmosphere!

I know child photography is hard and can imagine how many takes you would have had to get before getting a nice shot. If as posed, had tilted her head to look directly at the camera, with that puppydog look, it would have made a pleasing shot.

I love seeing pregnant women. There is definitely something beautiful about the "lump". A different pose would have made it easier to tell what this image was of (other than the caption). Next time see if you can get rid of the distracting horizontal waistband mark (either get her to wear lower riding pants/skirt or wait until the skin rebounds).
